Apply for an AC declaration using this form. An AC declaration can help you if you want to work in a regulated profession in an EU country with your Dutch diploma.

Only fill out this form if you meet the following conditions:

  • You have a Dutch diploma for this profession.
  • You want to practice your profession in an EU country, Iceland, Liechtenstein, Norway or Switzerland.
  • Your profession is regulated in the country where you want to work.
